Effective July 1, 2019
POLIO
4 doses (3 doses if one given on or after the 4th BD)
DTaP
5 doses (4 doses if one given on or after the 4th BD)
HEP B
3 doses
MMR
2 doses (administered on or after the 1st BD)
VARICELLA
2 doses
Tdap
1 dose (required on or after the 7th BD)
ACCEPTABLE Immunization Documents:
- Yellow Immunization card
- MD/Healthcare Provider printout
- Copy of Blue CA Immunization Card must be signed and dated
- CAIR printout.

MEDICAL EXEMPTION – please visit CAIR-ME website.
Students MUST have all immunizations at the time of enrollment. Children behind on their required immunizations may be admitted conditionally if they are not currently due for any doses.
