Students needing to request transcripts for any reason must first pick up a transcript request form from the front office here at Antelope High School, both Mrs. Hardy and Mrs. Huntington will be happy to assist you with this. Or if you wish you may download the below form. If you are a former student of Antelope HS and reside out of the area, please fill out the transcript request form and email to: firstname.lastname@example.org or fax to 916-726-0700, please make sure to provide a complete address of where this transcript needs to be sent.
Transcript request forms require a parent signature unless the student is 18 years of age and student must present valid ID. There is a $3 charge for any copy of transcripts which must be paid first with Mrs. Carroll at the Student Services window, retain your receipt of payment and then turn in your blue transcript request form to the front office for Mrs. Cuneo to process. Please note any student with fines attached to their record will not have their transcript request processed until fines are paid for or books/items have been returned.
There is a 24 hour turn around from the time the request is turned in, on demand requests will not be granted. Be aware of deadlines on applications or letters you receive from colleges or organizations, they are very strict on U.S. Postmark deadlines. Students have two options on how transcripts are mailed, the first choice is that they can mail it themselves the second choice is that Antelope High School can mail it for them. Any student wishing Antelope High School to mail transcripts must provide Mrs. Scuka, the Registrar with the envelope(s) stamped and addressed to the organization or educational institution it must go to. If you are mailing more than 4 pieces of paper in one envelope extra postage will need to be put on the envelope you are submitting. Please note that all envelopes must be properly addressed or the U.S. Postal service will return all mail. Please see picture below for accuracy reference.
